Consent to Release Information

Gottlieb/West Towns Patients: If you have selected a new primary care physician (PCP), it is important to transfer your medical records. Your new physician will need these records to better understand you, your health history, and your current and future health needs.


It is better to transfer your records before you have your first visit with your new physician. You will need to go to your physician's office to sign a consent form authorizing the release of your medical record.

Please note: Consent forms may also be available at your new physician's office.
